{"id":73294,"date":"2026-05-08T18:27:49","date_gmt":"2026-05-08T12:57:49","guid":{"rendered":"https:\/\/www.nextias.com\/ca\/?p=73294"},"modified":"2026-05-08T18:28:29","modified_gmt":"2026-05-08T12:58:29","slug":"india-health-system-reforms","status":"publish","type":"post","link":"https:\/\/www.nextias.com\/ca\/current-affairs\/08-05-2026\/india-health-system-reforms","title":{"rendered":"Fixing Structural Deficits in India\u2019s Health System"},"content":{"rendered":"\n<p><strong>Syllabus: GS2\/ Governance\/ Health<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Context<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Recently the Minister of State for Health, informed Parliament that 43 new medical colleges have been established and <strong>11,682 MBBS seats<\/strong> along with <strong>8,967 postgraduate seats<\/strong> have been approved for the 2025-26 academic year.\n<ul class=\"wp-block-list\">\n<li>However, structural deficits<strong> <\/strong>continue to challenge India\u2019s health system.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Structural Deficits in India\u2019s Health System<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Massive Specialist Vacancies: <\/strong>According to Health Dynamics of India 2022\u201323, rural Community Health Centres (CHCs) face a <strong>specialist vacancy rate of nearly 79.9%.<\/strong>\n<ul class=\"wp-block-list\">\n<li>Only 4,413 specialists are available against the requirement of 21,964 specialists.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Private Sector in Medical Education:<\/strong> Of the 43 newly sanctioned medical colleges, 27 are in the private sector.\n<ul class=\"wp-block-list\">\n<li>Private medical colleges charge high tuition fees and capitation costs.<\/li>\n\n\n\n<li>Graduates from private institutions generally prefer <strong>urban private practice, <\/strong>as there is <strong>no binding obligation <\/strong>requiring them to serve in government hospitals or remote regions.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Dysfunctional Community Health Centres:<\/strong> A CHC serves as a <strong>first referral unit<\/strong> for a population of about<strong> 1.6 lakh to 2 lakh <\/strong>and is expected to have <strong>30 beds with five specialists; <\/strong>physician, surgeon, obstetrician, paediatrician, and anaesthetist.\n<ul class=\"wp-block-list\">\n<li>However, most CHCs remain non-functional due to specialist shortages.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Poor Working Conditions for Doctors: <\/strong>Doctors posted in remote regions often face lack of staff quarters, professional isolation, heavy workloads and limited career growth opportunities.<\/li>\n\n\n\n<li><strong>Faculty Shortages: <\/strong>Eleven out of 18 AIIMS report nearly <strong>40% vacancies<\/strong> in teaching and research faculty positions.<\/li>\n\n\n\n<li><strong>Imbalanced Health Budget Priorities:<\/strong> The Union health budget remains heavily focused on <strong>capital expenditure<\/strong> (infrastructure expansion), while critical operational components such as human resources, diagnostics, emergency care, and maintenance remain underfunded.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Significance of Strengthening Public Healthcare<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Social Justice and Equity: <\/strong>Public healthcare remains the primary source of medical care for poor and vulnerable populations. Weak public health systems deepen<strong> regional inequalities and social exclusion.<\/strong><\/li>\n\n\n\n<li><strong>Out-of-Pocket Expenditure:<\/strong> Strong public healthcare systems reduce dependence on expensive private healthcare. This lowers catastrophic health expenditure and medical indebtedness among households.<\/li>\n\n\n\n<li><strong>Universal Health Coverage:<\/strong> Strengthening public healthcare is essential for National Health Policy goals, and<strong> Sustainable Development Goal 3<\/strong> related to health and well-being.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Government Initiatives&nbsp;<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Pradhan Mantri Ayushman Bharat Health Infrastructure Mission (PM-ABHIM): <\/strong>The scheme designed to build critical healthcare infrastructure, particularly in rural areas, to manage future pandemics and strengthen surveillance.<\/li>\n\n\n\n<li><strong>Ayushman Bharat Programme: <\/strong>It aims to strengthen primary healthcare through <strong>Health and Wellness Centres (HWCs)<\/strong> and provide financial protection through Pradhan Mantri Jan Arogya Yojana (PM-JAY).<\/li>\n\n\n\n<li><strong>Telemedicine Initiatives under eSanjeevani: <\/strong>It<strong> <\/strong>promotes digital healthcare consultations to improve healthcare access in remote and underserved regions.<\/li>\n\n\n\n<li><strong>Pradhan Mantri Bhartiya Janaushadhi Pariyojana (PMBJP):<\/strong> Ensuring the availability of high-quality generic medicines at affordable prices through dedicated Kendra centers.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Measures to Strengthen India\u2019s Public Health System<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Align Medical Education with Public Health Needs:<\/strong> Government-sponsored postgraduate seats should be directly linked to specialist vacancies in <strong>Community Health Centres (CHCs)<\/strong> and district hospitals.\n<ul class=\"wp-block-list\">\n<li>Candidates receiving subsidised postgraduate training should be required to serve in designated public facilities after completion of training.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Rural Service with Incentives: <\/strong>Doctors serving in difficult areas should receive additional allowances, career advancement incentives, enhanced pension and service benefits.<\/li>\n\n\n\n<li><strong>Categorise Health Facilities:<\/strong> PHCs and CHCs should be classified into<strong> normal areas, difficult areas, <\/strong>and <strong>most difficult areas,<\/strong> as was done in <strong>Chhattisgarh under the Rural Medical Corps Scheme.\u00a0<\/strong>\n<ul class=\"wp-block-list\">\n<li>Special incentives should be linked to the difficulty level of postings.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>All or None principle:<\/strong> Specialists should be <strong>deployed as complete teams<\/strong> rather than scattered individually.\n<ul class=\"wp-block-list\">\n<li>Functional CHCs require the simultaneous availability of <strong>all five core specialists.<\/strong><\/li>\n\n\n\n<li>Concentrated deployment in selected CHCs can <strong>create effective referral centres <\/strong>at the sub-district level.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Healthcare Infrastructure:<\/strong> Priority investment should focus on Staff quarters, Labour rooms, <strong>Operation theatres, Intensive care units, Blood storage facilities,<\/strong> Telemedicine and digital health infrastructure etc.<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Concluding remarks<\/strong><\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>India\u2019s healthcare crisis is a deeper structural <strong>mismatch between medical education, workforce deployment, and public health priorities.\u00a0<\/strong><\/li>\n\n\n\n<li>India must <strong>adopt a comprehensive strategy<\/strong> that integrates <strong>medical education with public service obligations<\/strong>, strengthens rural healthcare infrastructure, and creates a motivated health workforce capable of delivering equitable healthcare to all sections of society.<\/li>\n<\/ul>\n\n\n\n<p><strong>Source:
