Expansion of Health Insurance<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Universal Health Care (UHC), as defined by the <strong>Bhore Committee in 1946<\/strong>, calls for <strong>quality health care for all, irrespective of ability to pay<\/strong>.\n<ul class=\"wp-block-list\">\n<li>However, India remains far from this vision after nearly eight decades.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Instead, an illusion has been created that expanding <strong>health insurance schemes<\/strong> such as <strong>Pradhan Mantri Jan Arogya Yojana (PM-JAY)<\/strong> and <strong>State Health Insurance Programmes (SHIPs)<\/strong> will deliver <a href=\"https:\/\/www.nextias.com\/ca\/editorial-analysis\/24-03-2025\/universal-equitable-health-coverage-need\"><strong>Universal Health Coverage (UHC)<\/strong><\/a>.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Expansion of Health Insurance<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Over the past decade, India has witnessed rapid growth in State-sponsored health insurance:\n<ul class=\"wp-block-list\">\n<li><strong>PMJAY<\/strong>: Covers 58.8 crore individuals, with an annual budget of \u20b912,000 crore.<\/li>\n\n\n\n<li><strong>SHIPs<\/strong>: Cover a similar number, costing around \u20b916,000 crore.<\/li>\n\n\n\n<li><strong>Combined Spending<\/strong>: About \u20b928,000 crore in 2023\u201324, still a small share of overall health expenditure but rising steadily.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>They fall short of establishing a sustainable UHC framework, while these schemes provide short-term relief to patients.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Major Fault Lines in Health Insurance<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Underinvestment in Public Health: <\/strong>India\u2019s reliance on insurance reflects decades of neglect of public health infrastructure.\n<ul class=\"wp-block-list\">\n<li>In 2022, <strong>public expenditure on health was just 1.3% of GDP<\/strong>, far below the world average of 6.1%.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Promotion of For-Profit Medicine: <\/strong>Two-thirds of PMJAY\u2019s funds flow to private hospitals, reinforcing profit-driven care.\n<ul class=\"wp-block-list\">\n<li>Poor regulation of private providers exacerbates this imbalance, with little impact on hospitalisation rates but higher reliance on private hospitals.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Bias Towards Hospitalisation: <\/strong>Insurance schemes cover <strong>only in-patient care, neglecting primary and outpatient services<\/strong>, which are crucial for preventive and accessible health care.\n<ul class=\"wp-block-list\">\n<li>With an ageing population, costly tertiary care risks consuming most of the health budget while primary needs remain unmet.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Low Utilisation Despite High Coverage: <\/strong>Although official figures suggest 80% population coverage, utilisation is weak.\n<ul class=\"wp-block-list\">\n<li>Only about <strong>35% of insured hospital patients<\/strong> were able to use insurance in 2022\u201323.<\/li>\n\n\n\n<li>Lack of awareness, procedural hurdles, and resistance from private hospitals contribute to this gap.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Discrimination in Treatment: <\/strong>Private hospitals often prefer uninsured patients, who pay higher rates, while public hospitals may prioritise insured patients for revenue.\n<ul class=\"wp-block-list\">\n<li>It fosters <strong>discriminatory treatment<\/strong> and pressure to enrol patients on the spot.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Provider Dissatisfaction and Delays: <\/strong>Hospitals face <strong>low reimbursements<\/strong> and <strong>delayed payments<\/strong>.\n<ul class=\"wp-block-list\">\n<li>Pending dues under PMJAY alone exceeded \u20b912,000 crore in 2023, leading to hospital withdrawals and service suspensions.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li><strong>Corruption and Fraud: <\/strong>Fraudulent practices are rampant, including denial of care, inflated billing, and unnecessary procedures.\n<ul class=\"wp-block-list\">\n<li>Thousands of hospitals have faced action under PMJAY, but transparency remains weak, with little evidence of effective audits.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Learning from Global Experiences<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Countries like <strong>Canada and Thailand<\/strong> use social health insurance within a robust public health system. Key differences include:\n<ul class=\"wp-block-list\">\n<li><strong>Universal coverage;<\/strong><\/li>\n\n\n\n<li><strong>Focus on non-profit providers;<\/strong><\/li>\n\n\n\n<li><strong>Strong regulation and accountability<\/strong><\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>India\u2019s insurance schemes lack these safeguards, making them ill-suited to deliver true UHC.\n<ul class=\"wp-block-list\">\n<li>Earlier, IRDAI has announced a goal <strong>to make health insurance universal by 2047<\/strong>, aligning with India\u2019s centennial independence milestone.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Conclusion<\/strong><\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>India\u2019s path to Universal Health Care (UHC) cannot rest on insurance schemes alone, as they promote profit-driven hospitalisation, suffer from low utilisation, and remain vulnerable to corruption.\n<ul class=\"wp-block-list\">\n<li>It needs to be grounded in accessible, quality care for all.<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>There is a need to:\n<ul class=\"wp-block-list\">\n<li><strong>Increased public investment<\/strong> in health (above the current 1.3% of GDP).<\/li>\n\n\n\n<li><strong>Strengthening of primary and public health facilities<\/strong>.<\/li>\n\n\n\n<li><strong>Effective regulation of private providers<\/strong>.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<figure class=\"wp-block-table\"><table class=\"has-background has-fixed-layout\" style=\"background-color:#fff2cc\"><tbody><tr><td><strong>Daily Mains Practice Question<\/strong><br><strong>[Q]<\/strong> Critically examine the role of health insurance in improving healthcare accessibility and equity in India.
