Healthcare has become<\/span><\/span><\/span><\/span> one of India\u2019s largest sectors,<\/strong><\/span><\/span><\/span><\/span> both in terms of <\/span><\/span><\/span><\/span>revenue and employment<\/strong><\/span><\/span><\/span><\/span>.\u00a0<\/span><\/span><\/span><\/span><\/li>\n
The Indian healthcare sector is<\/span><\/span><\/span><\/span> growing at a brisk pace <\/strong><\/span><\/span><\/span><\/span>due to its strengthening coverage, services and increasing expenditure by public as well private players.<\/span><\/span><\/span><\/span><\/li>\n
India\u2019s healthcare delivery system is categorised into two major components public and private.\u00a0<\/span><\/span><\/span><\/span>\n
\n
The government,<\/strong><\/span><\/span><\/span><\/span> i.e. public healthcare system, comprises limited secondary and tertiary care institutions in key cities and focuses on providing basic healthcare facilities in the form of primary healthcare centres (PHCs) in rural areas.\u00a0<\/span><\/span><\/span><\/span><\/li>\n
The private sector <\/strong><\/span><\/span><\/span><\/span>provides a majority of secondary, tertiary, and quaternary care institutions with major concentration in metros and tier-I and tier-II cities.<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n
The report highlights<\/u><\/strong><\/span><\/span><\/span><\/span><\/p>\n

The report highlights two facts about the<\/span><\/span><\/span><\/span> Pradhan Mantri Jan Aarogya Yojana (PMJAY) scheme.<\/strong><\/span><\/span><\/span><\/span>\u00a0<\/span><\/span><\/span><\/span>\n
\n
It highlights the key <\/span><\/span><\/span><\/span>role of the treating doctor<\/strong><\/span><\/span><\/span><\/span> in deciding the type of medical package to be booked for a patient, and whether a patient will be registered under a PMJAY package at all.\u00a0<\/span><\/span><\/span><\/span><\/li>\n
The report <\/span><\/span><\/span><\/span>reveals how a doctor can mislead<\/strong><\/span><\/span><\/span><\/span> the patient on the premise that \u201cAyushman Bharat Clearance would take months.\u201d<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n
Contrary to popular perception<\/strong><\/span><\/span><\/span><\/span>, the time taken to settle claims in public facilities was not unduly high.\u00a0<\/span><\/span><\/span><\/span>\n

Of the claims registered across all facilities, <\/span><\/span><\/span><\/span>54 percent were settled.<\/strong><\/span><\/span><\/span><\/span> The average time taken for settlement of these claims was<\/span><\/span><\/span><\/span> about 21 days<\/strong><\/span><\/span><\/span><\/span>.<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n
There were<\/span><\/span><\/span><\/span> indications of a lack of active interest in the scheme<\/strong><\/span><\/span><\/span><\/span> by the medical team in public facilities.<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n

The scheme was<\/span><\/span><\/span><\/span> launched in September 2018 <\/strong><\/span><\/span><\/span><\/span>and recommended by the National Health Policy 2017, to achieve the <\/span><\/span><\/span><\/span>vision of Universal Health Coverage (UHC)<\/strong><\/span><\/span><\/span><\/span>.<\/span><\/span><\/span><\/span><\/li>\n
It is a <\/span><\/span><\/span><\/span>Centrally Sponsored Scheme<\/strong><\/span><\/span><\/span><\/span> having a central sector component under <\/span><\/span><\/span><\/span>Ayushman Bharat Mission<\/strong><\/span><\/span><\/span><\/span>.<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n

To<\/span><\/span><\/span><\/span> accelerate health system preparedness<\/strong><\/span><\/span><\/span><\/span> for immediate responsiveness for early prevention, detection and management, with a focus on <\/span><\/span><\/span><\/span>health infrastructure development<\/strong><\/span><\/span><\/span><\/span> including for Paediatric Care and with measurable outcomes.<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n
Key Features:\u00a0<\/strong><\/span><\/span><\/span><\/span>\n

It is the<\/span><\/span><\/span><\/span> world\u2019s largest health insurance\/ assurance scheme<\/strong><\/span><\/span><\/span><\/span> fully financed by the government and the cost of implementation is shared between the Central and State Governments.<\/span><\/span><\/span><\/span><\/li>\n
It <\/span><\/span><\/span><\/span>provides a cover of Rs. 5 lakhs per family per year <\/strong><\/span><\/span><\/span><\/span>for <\/span><\/span><\/span><\/span>secondary and tertiary care hospitalization <\/strong><\/span><\/span><\/span><\/span>across public and private empanelled hospitals in India.<\/span><\/span><\/span><\/span><\/li>\n
It covers<\/span><\/span><\/span><\/span> up to 3 days of pre-hospitalization<\/strong><\/span><\/span><\/span><\/span> and <\/span><\/span><\/span><\/span>15 days of post-hospitalization expenses <\/strong><\/span><\/span><\/span><\/span>such as diagnostics and medicines.<\/span><\/span><\/span><\/span><\/li>\n
There is no restriction on the family size, age or gender.<\/span><\/span><\/span><\/span><\/li>\n
The RSBY had a family cap of five members.\u00a0<\/span><\/span><\/span><\/span><\/li>\n
All pre-existing conditions are covered from day one.<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n

The households included are based on the deprivation and occupational criteria of <\/span><\/span><\/span><\/span>Socio-Economic Caste Census 2011 (SECC 2011) <\/strong><\/span><\/span><\/span><\/span>for rural and urban areas respectively.\u00a0<\/span><\/span><\/span><\/span><\/li>\n
The coverage mentioned under PM-JAY, therefore, also includes families that were covered in RSBY but are not present in the SECC 2011 database.\u00a0<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n

A host of factors within a public facility contribute to this lack of active interest by the medical team in the scheme.\u00a0<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n

Inadequate incentives for doctors:<\/strong><\/span><\/span><\/span><\/span>\n
\n
As per the guidelines of the National Health Authority (NHA), a percentage share of the claim revenues transferred by the state health agency to public facilities is to be distributed among the medical personnel as<\/span><\/span><\/span><\/span> staff incentives<\/strong><\/span><\/span><\/span><\/span>.\u00a0<\/span><\/span><\/span><\/span><\/li>\n
This share could vary across states. Consequently, a treating doctor also receives a financial incentive.\u00a0<\/span><\/span><\/span><\/span><\/li>\n
Yet, it <\/span><\/span><\/span><\/span>may not incentivise a doctor adequately<\/strong><\/span><\/span><\/span><\/span> to register a patient in the scheme.\u00a0<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n
Inadequate incentives for Arogyamitra:<\/strong><\/span><\/span><\/span><\/span>\n

With relatively modest physical infrastructure and human resources, the medical team was often stretched with clinical activities.\u00a0<\/span><\/span><\/span><\/span><\/li>\n
The only supporting staff for the scheme was an <\/span><\/span><\/span><\/span>Arogyamitra, whose responsibility was to register patients under an appropriate package<\/strong><\/span><\/span><\/span><\/span> in consultation with the treating doctor.\u00a0<\/span><\/span><\/span><\/span><\/li>\n
Arogyamitra’s<\/span><\/span><\/span><\/span> remuneration was linked <\/strong><\/span><\/span><\/span><\/span>to the number of cases he can successfully register under the scheme (pre-authorisations), and not to the final settlement of claims.\u00a0<\/span><\/span><\/span><\/span><\/li>\n
As a result, the<\/span><\/span><\/span><\/span> Arogyamitra had little incentive to follow-up the claims<\/strong><\/span><\/span><\/span><\/span> with the required documentation at subsequent stages and ensure settlement.<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n
Vested interests:<\/strong><\/span><\/span><\/span><\/span>\n

The<\/span><\/span><\/span><\/span> ignorance of the poor and the information asymmetry <\/strong><\/span><\/span><\/span><\/span>between doctors and patients creates a fertile ground for denying benefits to the poor to serve vested interests.\u00a0<\/span><\/span><\/span><\/span><\/li>\n
District-level aggregate figures<\/strong><\/span><\/span><\/span><\/span> indicated that the proportion of claims settled in public facilities was significantly lower than their private counterparts.<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n

An active interest in the scheme by public facilities can ensure a substantial volume of additional revenues, which could then be utilised for <\/span><\/span><\/span><\/span>infrastructure development and establishment<\/strong><\/span><\/span><\/span><\/span> of better amenities setting in a virtuous cycle.\u00a0<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n
Improving infrastructure:<\/strong><\/span><\/span><\/span><\/span>\n

The improved infrastructure could enhance the facilities\u2019 potential to cater to more packages and ultimately improve health coverage for the poor. <\/span><\/span><\/span><\/span><\/li>\n
This is in addition to ensuring that no poor person is excluded due to database errors in eligibility.\u00a0<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n
Role of governments:<\/strong><\/span><\/span><\/span><\/span>\n

Besides, state governments must play a complementary role by providing adequate manpower and enforcing accountability to ensure a higher volume of services in public facilities.<\/span><\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n
