The Federal Government is composed of three distinct branches: <\/span><\/span><\/span>legislative, executive, and judicial,<\/strong><\/span><\/span><\/span> whose powers are vested by the U.S. Constitution in the <\/span><\/span><\/span>Congress, the President, and the Federal courts, respectively.<\/strong><\/span><\/span><\/span><\/li>\n<\/ul>\n
What is the US debt ceiling?<\/u><\/strong><\/span><\/span><\/span><\/p>\n

The debt ceiling, or debt limit, is the<\/span><\/span><\/span> total amount the US government is allowed to borrow to finance its expenditure,<\/strong><\/span><\/span><\/span> such as paying salaries and welfare allowances.<\/span><\/span><\/span><\/li>\n
The debt limit was <\/span><\/span><\/span>introduced in 1917, <\/strong><\/span><\/span><\/span>when the US entered World War I.\u00a0<\/span><\/span><\/span><\/li>\n
Constitutionally, <\/span><\/span><\/span>Congress controls the government\u2019s expenditure.<\/strong><\/span><\/span><\/span> The debt ceiling was introduced in order to make it easier for the <\/span><\/span><\/span>executive to operate without having to turn to Congress every time it wanted to spend<\/strong><\/span><\/span><\/span> \u2014 it allowed the government to borrow as required as long as it kept under the <\/span><\/span><\/span>debt limit approved by Congress.<\/strong><\/span><\/span><\/span><\/li>\n
Currently, this limit is at <\/span><\/span><\/span>$31.4 trillion.<\/strong><\/span><\/span><\/span> <\/span><\/span><\/span>The Republicans,<\/strong><\/span><\/span><\/span> who have a majority in the House, are refusing to raise it unless the Democrat-run government agrees to their demands, which include a <\/span><\/span><\/span>significant cut in spending.<\/strong><\/span><\/span><\/span><\/li>\n<\/ul>\n
What happens if the govt defaults?<\/u><\/strong><\/span><\/span><\/span><\/p>\n

The government would no longer have the <\/span><\/span><\/span>money to function,<\/strong><\/span><\/span><\/span> and would have to decide who gets salaries, and how much.<\/span><\/span><\/span><\/li>\n
If Congress fails to increase the debt limit, it would cause severe <\/span><\/span><\/span>hardship to American families, harm our global leadership position, and raise questions about our ability to defend our national security interests.<\/strong><\/span><\/span><\/span><\/li>\n
Analysts say the <\/span><\/span><\/span>dollar would weaken,<\/strong><\/span><\/span><\/span> the <\/span><\/span><\/span>stock markets would collapse, and millions might lose their jobs<\/strong><\/span><\/span><\/span>. Also, the US\u2019s credit rating would be downgraded, making future borrowing more expensive.<\/span><\/span><\/span><\/li>\n<\/ul>\n
Measures adopted by US to Avert the Crisis :\u00a0<\/u><\/strong><\/span><\/span><\/span><\/p>\n
President Joe Biden and Republican House Speaker Kevin McCarthy agreed on a deal that can potentially avert the crisis.<\/span><\/span><\/span><\/p>\n

A cap and a raise: <\/strong><\/span><\/span><\/span>Under the deal, the <\/span><\/span><\/span>$31.4 trillion debt ceiling will be suspended until January 2025<\/strong><\/span><\/span><\/span> \u2014 until after Biden\u2019s current term. Until then, the government can keep borrowing to fund itself.<\/span><\/span><\/span>\u00a0<\/span><\/span><\/span>\n

In return, the White House has agreed to<\/span><\/span><\/span> cap non-defence discretionary spending at 2023 levels in 2024<\/strong><\/span><\/span><\/span>, and increase it by 1% the year after.<\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n
Work requirements made stricter: <\/strong><\/span><\/span><\/span>US president has agreed to increase work requirements for those who avail of government food stamps. The Supplemental Nutrition Assistance Program (SNAP) currently allows able-bodied <\/span><\/span><\/span>people above 50<\/strong><\/span><\/span><\/span> with no dependents to get the food stamps for three months \u201cduring any 36-month period when they cannot show they are employed or in a work or training program for at least 20 hours a week.<\/span><\/span><\/span>\n

Under the new deal,<\/span><\/span><\/span> this age limit will be raised to 54. Veterans and the homeless are exempt from this.<\/strong><\/span><\/span><\/span>\u00a0<\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n

According to <\/span><\/span><\/span>Article 112<\/strong><\/span><\/span><\/span> of the Indian Constitution, the annual financial statement of a year is a statement of the estimated receipts and expenditure of the government for that particular year((April 1st to March 31st).<\/span><\/span><\/span>\n

The term <\/span><\/span><\/span>\u2018Union Budget\u2019<\/strong><\/span><\/span><\/span> is <\/span><\/span><\/span>nowhere mentioned <\/strong><\/span><\/span><\/span>in the Indian Constitution.\u00a0<\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n
The President of India<\/strong><\/span><\/span><\/span> is required to cause the annual financial statement to be laid before the Parliament.<\/span><\/span><\/span><\/li>\n
Article 266<\/strong><\/span><\/span><\/span> of the Constitution requires the government to credit all revenues received by it, including taxes and other revenues, into the Consolidated Fund of India, unless otherwise provided by law.<\/span><\/span><\/span><\/li>\n

Over the years, there have been several episodes of physical and verbal violence.<\/span><\/span><\/span><\/li>\n
Each episode has the people at large passionately arguing the issue for a few days, only for their words to fade away until the next brutal incident.\u00a0<\/span><\/span><\/span><\/li>\n
Doctors also stage protests but are ultimately honor-bound to return to duty, to reprise their roles as healers.\u00a0<\/span><\/span><\/span><\/li>\n
Thus, the issue remains largely unaddressed.<\/span><\/span><\/span><\/li>\n<\/ul>\n

According to a systematic review of recent research published in the Journal of Postgraduate Medicine in July 2020, \u201cIn developing countries, more than<\/span><\/span><\/span> 50% of doctors have faced patient-led verbal and physical abuse.<\/strong><\/span><\/span><\/span>\n
\n
Such violence has been on a rising trend in economically developing countries and is lowest in developed ones.\u00a0<\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n
Workplace violence is mostly directed at junior doctors and residents, with the incidence progressively dropping against more senior healthcare workers.\u00a0<\/span><\/span><\/span><\/li>\n
The violence also happens more often in high-stake settings, such as in the emergency wing and intensive care units, or in departments with patients who have compromised mental faculties.<\/span><\/span><\/span><\/li>\n
The perpetrators of workplace violence against physicians are family members or relatives of the patient in <\/span><\/span><\/span>82.2% of case<\/strong><\/span><\/span><\/span>s, as per a paper published in PLoS ONE in 2020.\u00a0<\/span><\/span><\/span><\/li>\n
Studies also show that female medical professionals, with lesser years of experience, are objectively more at risk of being on the receiving end of both physical and verbal workplace violence.<\/span><\/span><\/span><\/li>\n
According to a 2016 paper in the National Medical Journal of India, <\/span><\/span><\/span>75% of the violence against doctors is verbal<\/strong><\/span><\/span><\/span>, including intimidation and threats.\u00a0<\/span><\/span><\/span><\/li>\n
Most doctors who suffer from them never report them; some notify their department seniors but are often told that these experiences are \u201cpart of the job\u201d.\u00a0<\/span><\/span><\/span><\/li>\n

Often, patients or their relatives turn to violence because the healthcare system hasn\u2019t met their expectations.<\/span><\/span><\/span><\/li>\n
Some perpetrators become violent over concerns about the patient\u2019s condition, such as actual or perceived deterioration of their condition or doubts about the wrong treatment being administered.\u00a0<\/span><\/span><\/span><\/li>\n
Some others become violent due to issues such as high payment dues and protracted waiting times.<\/span><\/span><\/span><\/li>\n<\/ul>\n

Psychological Impact: <\/strong><\/span><\/span><\/span>Being on the receiving end of verbal or physical violence has an <\/span><\/span><\/span>immense psychological impact.\u00a0<\/strong><\/span><\/span><\/span>\n

Some studies have reported symptoms of<\/span><\/span><\/span> post-traumatic stress disorder, anxiety, and depression in doctor<\/strong><\/span><\/span><\/span>s who have faced violence from patients or their kin.<\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n
Effect Rural Healthcare:<\/strong><\/span><\/span><\/span> India has a <\/span><\/span><\/span>skewed doctor-patient ratio<\/strong><\/span><\/span><\/span> (1:854, including Ayurveda and homeopathy practitioners), doctors often decide to operate in resource-abundant settings for their own security. This in turn affects rural healthcare.<\/span><\/span><\/span><\/li>\n
Effect Quality of Healthcare:<\/strong><\/span><\/span><\/span> After experiencing violence, the PLoS ONE study found that doctors <\/span><\/span><\/span>wish to stop offering emergency services,<\/strong><\/span><\/span><\/span> refer patients sooner to more specialists, and <\/span><\/span><\/span>over-investigate symptoms and prescribe more tests.\u00a0<\/strong><\/span><\/span><\/span>\n

They also tend to offer <\/span><\/span><\/span>less lifesaving medical and surgical interventions<\/strong><\/span><\/span><\/span> that a patient may require, over fear that a risky procedure may provoke violent action. At the same time, measures to mitigate workplace violence towards them must be multi-pronged and focus on institutional and policy measures.<\/span><\/span><\/span><\/li>\n
Ensuring that there are counselors to help patients and relatives in times of high emotional distress (together with people who can translate between languages) can eliminate any miscommunication regarding a patient\u2019s condition and treatment.<\/span><\/span><\/span><\/li>\n
\u00a0In addition, a robust security system and not allowing more than a few relatives by a patient\u2019s bedside may also be important \u2014 as well as enforcing the law well enough to bring perpetrators to book in a reasonable span of time.<\/span><\/span><\/span><\/li>\n

The Union Health Ministry recently urged State governments to audit hysterectomy trends in public and private hospitals, in response to a Supreme Court petition arguing that women from marginalised locations are at risk of unjustified hysterectomies for economic gains and exploitation.\u00a0<\/span><\/span><\/span><\/p>\n

The Court also gave a three-month deadline to States, directing them to implement the guidelines previously issued by the Centre.<\/span><\/span><\/span><\/li>\n<\/ul>\n
About Hysterectomy\u00a0<\/u><\/strong><\/span><\/span><\/span><\/p>\n

Hysterectomy is an operation to remove the uterus.<\/span><\/span><\/span><\/li>\n
the criteria for getting a hysterectomy: <\/strong><\/span><\/span><\/span>The highest percentage of hysterectomies were to treat excessive menstrual bleeding or pain (51.8%); 24.94% for fibroids; 24.94% for cysts and 11.08% for uterine disorder or rupture, according to NFHS-5 data. Yet, studies have shown that \u201cmany of these causes were considered to be treatable and surgery could be avoided\u201d.\u00a0<\/span><\/span><\/span><\/li>\n<\/ul>\n

The procedure can easily be misused by either private clinics who earn profits (from insurance money) or by contractors in unorganised sectors such as the sugar-cane-cutting industry, where \u2018wombless women\u2019 are the norm to eliminate the need for menstrual care and hygiene among workers.<\/span><\/span><\/span><\/li>\n
despite the provisions, private hospitals in Bihar, Chhattisgarh and Rajasthan engaged in unethical practices, unnecessary procedures and did not inform women of side effects or take their informed consent.\u00a0<\/span><\/span><\/span><\/li>\n
In doing so, they \u201cfailed in providing and regulating constitutionally mandated reproductive healthcare to women\u201d and violated their \u201crights to health, bodily integrity and informed consent<\/span><\/span><\/span><\/li>\n<\/ul>\n

Hysterectomies may cause long-term injuries and disabilities, requiring follow-up and post-operative care, both rarely available or affordable.\u00a0<\/span><\/span><\/span><\/li>\n
In some cases, where hysterectomies are not justified yet still done, women may continue to suffer post-surgery and might need additional surgery.<\/span><\/span><\/span><\/li>\n<\/ul>\n
Governments Step<\/u><\/strong><\/span><\/span><\/span><\/p>\n

The Union Health Ministry in 2022 issued guidelines to prevent unnecessary hysterectomies \u2014 listing possible indications of when hysterectomy may be required and alternative clinical treatments for gynaecological issues.<\/span><\/span><\/span>\n
\n
\u00a0Further, they recommended setting up district, State-level and national hysterectomy monitoring committees to monitor and collect data on age, mortality, and occupations, among other details.\u00a0<\/span><\/span><\/span><\/li>\n<\/ul>\n<\/li>\n
The monitoring committees are also tasked with creating awareness, among both practitioners and patients, about bodily anatomy, the role of uterus and when hysterectomies are actually indicated.<\/span><\/span><\/span><\/li>\n
The government\u2019s flagship health insurance programme, the Ayushman Bharat Pradhan Mantri Jan Arogya Yojana provides health cover of \u00e2\u201a\u00b95 lakh for 1,949 procedures, including hysterectomies.\u00a0<\/span><\/span><\/span><\/li>\n
The government has authorised 45,434 hospitals to conduct these operations.\u00a0<\/span><\/span><\/span><\/li>\n<\/ul>\n
