{"id":14058,"date":"2021-02-23T00:00:00","date_gmt":"2021-02-23T00:00:00","guid":{"rendered":"https:\/\/www.nextias.com\/current_affairs\/uncategorized\/23-02-2021\/an-estimate-of-wash-across-healthcare-facilities-in-india\/"},"modified":"2021-02-23T00:00:00","modified_gmt":"2021-02-23T00:00:00","slug":"an-estimate-of-wash-across-healthcare-facilities-in-india","status":"publish","type":"post","link":"https:\/\/www.nextias.com\/ca\/current-affairs\/23-02-2021\/an-estimate-of-wash-across-healthcare-facilities-in-india","title":{"rendered":"An estimate of WASH across healthcare facilities in India"},"content":{"rendered":"<p><strong>In News: <\/strong>Researchers from the <strong>Center for Disease Dynamics, Economics and Policy (CDDEP)<\/strong>, Maryland, US, have estimated the <strong>cost of ensuring WASH. <\/strong><\/p>\n<p><strong>Key Findings <\/strong><\/p>\n<ul>\n<li>It estimates that i<strong>mproving and maintaining WASH across the public healthcare facilities<\/strong> in India <strong>for a year would cost $354 million<\/strong> (Rs 2567,00,00,000 approximately) in capital costs and <strong>$289 million (Rs 2095,00,00,000 approximately)<\/strong> in <strong>recurrent expenses.<\/strong><\/li>\n<li>The study further finds that the <strong>most costly interventions<\/strong> were <strong>providing clean water,<\/strong> <strong>linen reprocessing<\/strong> and <strong>sanitation while the least expensive <\/strong>was <strong>hand hygiene, medical device reprocessing and environmental surface cleaning. <\/strong><\/li>\n<li>This study was a part of a larger project to determine the <strong>cost-effectiveness of WASH <\/strong>interventions to reduce <strong>healthcare-associated infections<\/strong> among mother and neonates across the Indian healthcare system.<\/li>\n<\/ul>\n<table border=\"1\" cellspacing=\"0\" style=\"width:700px\">\n<tbody>\n<tr>\n<td style=\"vertical-align:top; width:468.0pt\">\n<p><strong>Other reports <\/strong><\/p>\n<ul>\n<li>A 2019 joint global baseline report by WHO and UNICEF had pointed out that globally, <strong>one in four healthcare facilitie<\/strong>s <strong>lacked basic water servicing<\/strong> and one in five had no <strong>sanitation service and 42% had no hygiene facilities at point of care.<\/strong><\/li>\n<li>A WHO document on WASH in healthcare facilities points out that 8,27,000 people in <strong>low- and middle-income countrie<\/strong>s die as a result of <strong>inadequate water, sanitation <\/strong>and <strong>hygiene each year.<\/strong> Also, the death of<strong> 2,97,000 children under five years<\/strong> can be <strong>prevented each year if better WASH could be provided<\/strong>.<\/li>\n<\/ul>\n<ul>\n<li>On a positive note, a 2<strong>012 WHO report had calculated that for every dollar <\/strong>invested in sanitation, <strong>there was $5.50 to be gained in lower health costs<\/strong>, more productivity and fewer premature deaths.<\/li>\n<\/ul>\n<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p><strong>Relevance of the study<\/strong><\/p>\n<ul>\n<li>The India study by CDDEP comes as a welcome first-level estimate as ensuring the <strong>availability and sustainable management of water<\/strong> and <strong>sanitation<\/strong> to all which is one of the <strong>2030 sustainable development goals of the WHO.<\/strong><\/li>\n<li>It <strong>offers policymakers an opportunity to address multiple overlapping problems through interventions on WASH in healthcare facilities.<\/strong><\/li>\n<li>This study forms the starting point for larger <strong>costing estimates, it also highlights<\/strong> the <strong>need for a concerted effort from local bodies, State and Central governments<\/strong> to <strong>sustainably address quality and inequality issues in WASH provision.<\/strong><\/li>\n<\/ul>\n<p><strong>About WASH<\/strong><\/p>\n<ul>\n<li>The term \u201c<strong>WASH in health care facilities\u201d<\/strong> refers to the<strong> provision of water, sanitation, <\/strong>health <strong>care waste management, hygiene<\/strong> and <strong>environmental cleaning infrastructure<\/strong>, and <strong>services across all parts of a facility. <\/strong><\/li>\n<li>\u201c<strong>Health care facilities\u201d<\/strong> encompass all <strong>formally recognized facilities <\/strong>that <strong>provide health care, including primary (health posts and clinics), secondary, and tertiary (district or national hospitals), public and private (including faith-run), and temporary structures designed for emergency contexts (e.g., cholera treatment centres)<\/strong>. They may be located in urban or rural areas.<\/li>\n<li>Across all regions, WASH services in health care facilities fall short of WHO and national standards.\n<ul>\n<li>The <strong>availability of WASH services,<\/strong> especially in <strong>maternity and primary care settings where they are often absent,<\/strong> <strong>supports core universal health care aspects of quality, equity, and dignity for all people. <\/strong><\/li>\n<\/ul>\n<\/li>\n<li>Basic WASH services in health care facilities are fundamental to <strong>providing quality care and for ensuring that primary health commitments, <\/strong>as detailed in the <strong>Astana Declaration, a<\/strong>re achieved.<\/li>\n<\/ul>\n<p><strong>Concerns<\/strong><\/p>\n<ul>\n<li>Inadequate Water Sanitation and Hygiene (WASH) services in India\u2019s health facilities, contribute to <strong>the high neonatal mortality rate, which is currently 24 deaths per 1000 live births. <\/strong><\/li>\n<li>This is primarily <strong>due to weak infection prevention <\/strong>and <strong>control in health facilities. Hospitals with poor WASH facilities <\/strong>are also found to have <strong>higher rates of maternal <\/strong>mortality.<\/li>\n<li>Due to the lack of <strong>access to water and sanitation, women are further discouraged from institutional delivery or they delay health-seeking advice.<\/strong><\/li>\n<li>Inadequacies in proving WASH and also lack of infection prevention and control can lead to <strong>healthcare-associated infections<\/strong>.\n<ul>\n<li>Some of the pathogens to look out for are <strong>Acinetobacter baumannii, Enterococcus faecalis, Escherichia coli, Salmonella typhi, Streptococcus pneumonia and many more. <\/strong>\u201c<\/li>\n<li>These pathogens are <strong>commonly implicated as causative agents <\/strong>of<strong> healthcare-associated infections because of their abilit<\/strong>y to<strong> develop resistance to antibiotics.<\/strong><\/li>\n<li>Common healthcare-associated infections include <strong>central-line-associated bloodstream infections, catheter-associated urinary tract infections, surgical site infections and ventilator-associated pneumonia.<\/strong><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>Steps taken <\/strong><\/p>\n<ul>\n<li><strong>KAYA KALP schem<\/strong>e &#8211;\n<ul>\n<li>In 2015, the Ministry of Health and Family Welfare (MoHFW) with support from <strong>UNICEF instituted the KAYAKALP schem<\/strong>e to r<strong>ecognise and reward <\/strong>the excellence of<strong> health facilities <\/strong>in<strong> promoting cleanliness<\/strong>, thereby improving the q<strong>uality of health care services<\/strong>.\u00a0<\/li>\n<\/ul>\n<\/li>\n<li><strong>Role of UNICEF<\/strong> &#8211;\n<ul>\n<li><strong>UNICEF<\/strong> works closely with the <strong>Government of India on increasing WASH <\/strong>in <strong>health care facilities<\/strong> and its approach is evidence-based.<\/li>\n<li>Their work includes <strong>gap assessment, demonstration of workable models <\/strong>on the ground and <strong>assistance in planning, management<\/strong> and <strong>monitoring to scale up successful institutional WASH models.<\/strong><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><a href=\"https:\/\/www.thehindu.com\/sci-tech\/science\/an-estimate-of-wash-across-healthcare-facilities-in-india\/article33889963.ece\" target=\"_blank\" rel=\"noopener\"><strong>Source :TH<\/strong><\/a><\/p>\n","protected":false},"excerpt":{"rendered":"<p>In News: Researchers from the Center for Disease Dynamics, Economics and Policy (CDDEP), Maryland, US, have estimated the cost of ensuring WASH.
